Abstract

Supernumerary teeth are teeth in addition to normal dentition. A mesiodens is a supernumerary tooth  located  in  the  upper  jaw,  in  the  midline  area,  generally  between  the  two  central  incisors,  presenting  in  a  single,  multiple  or  bilateral  way.  Although  they  are  often  asymptomatic,  some  complications  that  can  be  caused  include  impaction  of  permanent  incisors,  delayed  eruption,  ectopic  eruption,  crowding,  abnormal  root  formation,  middle  diastema,  cystic  lesions,  rotation  of  adjacent teeth, or root resorption. A case of inverted mesiodens in a nine-year-old female patient, referred for evaluation of a supernumerary tooth by cone beam computed tomography (CBCT) is presented. In transaxial views, the included mesiodens was observed in an inverted position, located centrally on the alveolar bone ridge, in the palatine vestibule direction between the anterior nasal spine and the nasopalatine canal towards palatine. Its coronary portion was closely related to the nasal floor and its radicular portion has a palatal curvature. The use of volumetric imaging studies such as CBCT allow to assess the location and precise relationship of these teeth with the adjacent anatomical structures, enabling an adequate planning of aesthetic and orthodontic treatment.
