The dentigerous cyst is derived f rom the dental follicle of an impacted or erupting tooth and is classified as a developmental cyst. A dentigerous cyst has been described that occurs as a result of an inflammatory process of a non-vital deciduous tooth, which involves the dental follicle of the underlying tooth, stimulating the accumulation of fluid and the separation between the reduced epithelium of the enamel and the crown. This entity is diagnosed early between the first and second decade of life, manifesting with pain and inflammation. This report describes the imaging evolution of a case of dentigerous cyst in a nine-year-old pediatric patient with emphasis on two-stage conservative surgical management, extraction of the deciduous tooth, decompression with a rigid catheter, and saline flushes for six months. and in the second intervention a total cystectomy, peripheral osteotomy and histopathological confirmation were performed. Clinical and radiographic follow-up was carried out for three years, evaluating the preservation of root growth of the 45 tooth to favor tooth eruption, until a functional position in the dental arch was achieved.
