Gubernacular canal associated with a retained tooth with a pericoronary image suggestive of a cystic lesion

Abstract

The  gubernacular  canal  (GC)  is  a  bony  canal,  part  of  the  gubernacular  dentis,  composed  of  fibrous connective tissue that contains epithelial cells, as well as peripheral nerves, blood, lymphatic vessels and remnants of lamina dura, it represents the passage of the tooth through the bone. The observation of GC in conventional 2D radiographic studies is inf requent, because these canals are very thin and are found in the cancellous bone, not related to the corticals of the jaws, the use of cone beam computed tomography (CBCT) has made possible your accurate assessment. This report describes a case of GC in a 13-year-old female patient who was referred for location of retained tooth 21 by CBCT. In multiplanar reconstructions, tooth 21 was retained in a vertical position with the crown between apical thirds of 11 and 22. A hypodense pericoronary image was observed, with corticalized borders suggestive of a cystic lesion, as well as a canal-shaped prolongation of approximately 8.12 mm, whose borders approached caudally, leading to the alveolar ridge, suggesting a GC. The CBCT allowed evaluating its limits, trajectory and relationship with the retained tooth
