Abstract

The  radicular  cyst  (RC)  constitutes  the  most  f requent  cystic  lesion  in  the  jaws,  it  has  a  higher  occurrence between the third and sixth decades of life, predominance in the male sex. Clinically, it  is  usually  asymptomatic  and  small  (0.5-1.5  cm  in  diameter),  it  rarely  presents  as  an  extensive  lesion. A case of CR in a 54-year-old male patient is describe, referred for presenting an increase in volume in the right mandibular region, symptomatic, with two years of evolution, without previous treatment. Upon evaluation by means of CBCT a hypodense image was observed, defined and corticalized  borders,  located  in  the  lower  right  molar  area,  in  association  with  the  apex  of  tooth  44, with previous endodontic treatment, which conditioned the thinning and fenestration of the vestibular cortex. The image was 22.64 mm high, 38.89 mm long and 18.50 mm wide. The clinical differential diagnosis was odontogenic keratocyst and ameloblastoma. The histopathological study of  the  specimen  obtained  by  incisional  biopsy  determined  that  it  was  RC.  Excision  of  the  lesion,  curettage and place platelet-rich fibrin concentrate was performed. Post-operative controls were established at 7, 14, and 21 days. Prosthetic rehabilitation is ongoing.
