The dental follicle (DF) is an ectomesenchymal tissue that surrounds the developing tooth germ. Under normal circumstances, the DF disappears after the eruption of the tooth, however, in cases of unerupted teeth, its remains persist within the maxilla and/or mandible in association with the unerupted tooth and may undergo morphological and molecular alterations. The case of a nine-year-old patient with absence of dental unit 21 in the oral cavity is described. The clinical, radiographic and histopathological evaluation reported that it was a hyperplastic FD. The importance of comprehensive evaluation to achieve a successful treatment is concluded
